Supported Living
We provide 24-hour support for people with learning disabilities in the comfort of their own homes. Our approach is person-centred, meaning everything we do is focused on what matters most to everyone. We aim to create a safe, warm, and homely environment where people feel empowered, supported, and in control of their own lives.
We work closely with each person to understand their goals, interests, and preferences. Whether it is daily routines like sleeping patterns, choosing male or female support workers, or exploring hobbies and new experiences, we ensure that every aspect of support is tailored to suit the individual.
Before support begins, we carry out an initial assessment to make sure we can meet the person’s needs. From there, we develop a personalised support plan together, ensuring the individual is fully involved and gives their consent at every stage. These plans focus on achieving meaningful outcomes and are reviewed regularly-at least once a month—to make sure they continue to reflect the person’s wishes and needs.
We also understand how important relationships are. We actively support people to build friendships and stay connected with their families, friends, partners, and neighbours, helping to reduce social isolation and promote a sense of belonging.
Above all, we respect each person’s individuality, dignity, and right to be part of their community. We support people to enjoy everyday life and social interaction just like anyone else. Our staff are trained to understand the emotional and physical impact of disability and illness, and we are committed to providing compassionate, respectful support at all times.
We currently provide support across three Supported Living homes and are proudly commissioned under the Durham County Council framework.
